$5,000,000 E&O Limit Cost $245.00
$2,000,000 CGL Limit Cost $100.00

Add Provincial Sales Tax –Newfoundland 15% Manitoba 8% to the total cost.

Additional Coverage

  • Criminal Defense up to $100,000.00
  • Disciplinary Defense up to $25,000
  • Additional cost for pretrial or appeal costs up to $250 per day
  • Abused Patient therapy costs up to $10,000 per day
  • Optional purchase of Commercial General Liability for $2,000,000

  • If you are paying by credit card or Electronic Fund Transfer, hit the submit button
  • If paying by cheque, print the application and mail it and the cheque back to us

About Flat Forms

A flat form is for use using Adobe Acrobat DC Fill & Sign tools.  Adobe Acrobat DC will generate a PDF attachment you can email.

A flat form does not have interactive fields. you can use Adobe Acrobat DC Fill & Sign tools to add text and other symbols anywhere on the form.
Note: For full instructions on how to fill & sign your forms electronically from your desktop using Acrobat DC or Acrobat Reader DC. Click here. You will need Acrobat Reader DC installed.

About Fillable Forms

An interactive or fillable form contains fields that you can select and fill in.  After completing your form you will need to "Output" your form before sending it. You "Output" your form by printing the completed form as a file and send it as an attachment (only if you use Acrobat DC to sign it) or you can print it on paper, sign the form and fax it or mail it. For fillable forms, the Fill & Sign tool displays only the Sign option if opened with Acrobat DC.

Important! Just saving the fillable form after filling it in will not save your information, you must "Output" your completed form.
You can print the form as a file. Windows users can print the application as a "Microsoft XPS Document". After printing as a file you can send your application as an attachment using your regular email program.
